Make a referral to the Mental Health Support Team

Thank you for considering making a referral to the Mental Health Support Team (MHST). Please note that the Mental Health Support Team is not a crisis service.

We are also not an eating disorder specialist service and would encourage you to book an appointment with your GP to discuss any difficulties around eating. You can also fill in an online self-referral to YOUnited for support with difficulties around eating.

Please note that the Mental Health Support Team is not a crisis service. If you feel you are in crisis or are concerned about someone else in crisis, we recommend that you:

contact the First Response Service by calling 111 and choose the mental health option or
visit your local hospital's accident and emergency (A&E) department

The referral form can be completed by yourself, with support from professionals or family if needed.

Please use this form to explain the reason you are making this referral. Your referral may not be seen by an MHST professional from our service for up to 7 days.

Before making a referral, please check that your school is covered by the Mental Health Support Team. If your school is not listed, please visit YOUnited - CPFT NHS Trust to complete the referral.

The referral may take around 15 to 20 minutes to complete. The information you provide helps us make sure that the child or young person gets the support which best meets their needs.

You cannot save the referral request once you have started.
You will need to provide some information in order to submit your referral. This includes:
- your full name, date of birth, home address and NHS number (if known)
- contact details
- details of your school
- the reason for this referral
The email address you provide must be used regularly. We will always attempt contact using the preferred contact method first.

I am a parent or carer

Complete this form if you would like support for a primary school aged child experiencing fears/worries/behavioural difficulties, or a secondary school aged young person experiencing worry and/or low mood. If their difficulty is around something else, or they do not attend a school supported by MHST, please complete a referral form to YOUnited.
