Welcome to our service

Our speech and language team aim to provide advice, help and support for children and young people aged 0 to 19 years old who have difficulties with speech, language, communication and swallowing.

More information about our services and offer

Parents and carers can use this information at home to support their child. Educational staff can also use this information with children in their school or setting.

Please try these ideas and advice before making a referral to the Speech and Language Therapy Service.

Referral forms and guidance

Pre-school children (Early advice sessions) 

Early advice sessions are for parents of toddlers and pre-school children aged between 2 and 5 years old. These are short 1-to-1 sessions to discuss your child’s speech and language concerns with a specialist. These should be completed before a referral. 

Referrals for children under 5 years old must be completed and submitted by a parent or carer.

Once a referral has been made, we will send you an email to book a video or telephone appointment. This appointment will be with a speech and language therapist. You will be able to talk about your concerns and what you would like help with.

Swallowing (dysphagia)

If you have any concerns about your child or young person's swallowing, eating and drinking skills please discuss with a healthcare professional or a educational professional. They can make a referral to the service for a feeding or swallowing assessment. 

This referral form should be completed by the healthcare professional or education staff.

Primary, secondary, college and home schooled children

Referrals for children and young people, up to 18 years old, must be completed and submitted by school staff.

We require different referral forms for different school types. Completed forms can be emailed to ccs-tr.slt@nhs.net. We will contact you to discuss the request for involvement.

  • If you are referring a child in a primary school, please complete a request for involvement (primary) form and discuss it with your named therapist.
  • If you are referring a child in a secondary school, please complete the secondary school referral form.
  • If you are referring a child in a home school, independent school or sixth-form college, please complete a request for involvement (college, dependent and home school) form.
