Mental Health Support Team
Make a referral
Before making a referral, please check that your school is covered by the Mental Health Support Team. If the school is not listed, please visit YOUnited - CPFT NHS Trust to complete the referral.
Complete this form if you would like support for a primary school aged child experiencing fears/worries/behavioural difficulties, or a secondary school aged young person experiencing worry and/or low mood.
If their difficulty is around something else, or they do not attend a school supported by MHST, please complete a referral form to YOUnited.
